
Millions of Indians who have kicked the habit may still carry a hidden risk. A new study published this week suggests that even after decades of being smoke-free, a person's lungs never fully recover from the damage caused by smoking.
The research, conducted by a team of international scientists, tracked thousands of former smokers over a 30-year period. It found that while quitting dramatically lowers the risk of lung cancer and chronic obstructive pulmonary disease (COPD), the risk never drops to the level of someone who has never smoked.
The study's key finding is that former smokers retain a significantly higher risk of developing COPD and lung cancer compared to lifelong non-smokers. The damage to lung tissue, particularly the small airways and alveoli, can remain for decades after the last cigarette.
“Quitting smoking is the single best thing a person can do for their lung health,” said the lead researcher. “But our data shows that the lungs carry a memory of that exposure. The risk of serious disease never returns to baseline.”
The researchers stress that this does not mean quitting is futile. On the contrary, the benefits are enormous. But it does mean that former smokers need to remain vigilant.
India is home to an estimated 120 million smokers, one of the largest populations in the world. While government campaigns have helped millions quit, this study highlights the need for long-term health monitoring for those who have successfully stopped.
Doctors recommend that anyone with a significant smoking history — even if they quit years ago — should undergo regular lung function tests and low-dose CT scans to catch any early signs of cancer or COPD. “A former smoker who feels fine should not assume their lungs are completely healthy,” a pulmonologist in Delhi commented. “Annual check-ups are crucial.”
The lungs have a remarkable ability to repair themselves after injury. But the chemicals in tobacco smoke cause permanent changes at the cellular level. Over time, these changes can lead to mutations that trigger cancer, or scarring that stiffens lung tissue.
The study also found that the longer and more heavily a person smoked, the greater the lasting damage. However, even light smokers who quit were found to have higher rates of lung disease than non-smokers.
The message from researchers is clear: quitting is essential, but it is not a cure-all. Former smokers must stay proactive about their health.
Looking ahead, scientists are exploring whether new therapies — such as anti-inflammatory drugs or regenerative treatments — could help repair the lingering damage in ex-smokers' lungs. For now, the best advice remains to never start smoking, and if you have quit, do not let your guard down.
