
The Chandipura virus, a lesser-known but deadly pathogen, has emerged as a critical public health threat in western India. With 22 children dead in Gujarat this monsoon alone, and cases surfacing in neighbouring Rajasthan, the Centre has swung into action, deploying a National Joint Outbreak Response Team to contain the spread.
The virus, named after the village in Maharashtra where it was first isolated in 1965, belongs to the Rhabdoviridae family—the same family as the rabies virus. It is transmitted primarily through the bite of infected sandflies, particularly the species Phlebotomus papatasi, and possibly some mosquito species.
Chandipura virus infection often begins with a sudden high fever, severe headache, and vomiting. In children—who are the most vulnerable—the disease can progress rapidly to encephalitis, or inflammation of the brain, within 24 to 48 hours.
As the infection worsens, patients may experience altered mental status, seizures, coma, and in severe cases, death. The incubation period is short, typically 1 to 4 days, which makes early detection difficult but critical.
Health officials have noted that the case fatality rate can be high, especially among children under 15. However, not every infected person develops severe illness; some may show only mild flu-like symptoms.
Chandipura virus has caused sporadic outbreaks in central and western India for decades. Notable outbreaks occurred in 2003 in Andhra Pradesh and Maharashtra, and again in Gujarat in 2004, when hundreds of children were affected. More recently, in 2019, Gujarat reported a small cluster of cases.
This year's monsoon has brought a particularly severe resurgence. Gujarat's State Health Minister confirmed that 22 children have died in the state this season, a toll that has triggered alarm bells. Rajasthan has also reported cases, though exact numbers have not been officially released.
The Centre's decision to deploy a National Joint Outbreak Response Team signals the seriousness of the situation. A multi-institutional probe, involving institutes such as the National Institute of Virology and the Indian Council of Medical Research, is now underway to understand the virus's spread and genetic makeup.
There is no specific antiviral drug or vaccine for Chandipura virus. Treatment is entirely supportive, focusing on managing symptoms and complications. Patients with severe encephalitis require hospitalisation, intravenous fluids, anticonvulsants to control seizures, and in critical cases, ventilator support.
Early hospitalisation significantly improves survival odds. Health experts stress that any child presenting with high fever and neurological symptoms during outbreak periods should be treated as a suspected case and referred to a tertiary care centre without delay.
Fluid and electrolyte balance is crucial, as is monitoring for raised intracranial pressure. The absence of a targeted therapy means that the outcome largely depends on the speed of medical intervention and the patient's immune response.
Since the virus is vector-borne, prevention hinges on controlling sandfly populations and avoiding bites. Health departments in affected districts have intensified fogging and insecticide spraying, especially in and around homes, schools, and cattle sheds—sandflies breed in cracks and damp soil.
Individuals, particularly in rural areas, are advised to use insect repellents, sleep under insecticide-treated bed nets, and wear full-sleeved clothing during dawn and dusk when sandflies are most active. Keeping surroundings clean, clearing vegetation, and plastering wall cracks can reduce breeding sites.
Public health campaigns have been launched to educate communities about symptoms and the importance of seeking immediate medical care. Surveillance systems have been strengthened to detect cases early and trace contacts.
Officials have not yet confirmed the total number of cases across both states, and the probe is expected to provide a clearer picture in the coming weeks. The monsoon season, which typically extends into September, poses an ongoing risk of further transmission.
With the national team now on the ground, the immediate focus is on containment and case management. The multi-institutional study may also inform future vaccine development, but for now, vigilance and vector control remain the only shields against this deadly virus.
