For a significant subset of patients, the headache is preceded or accompanied by neurological symptoms known as aura—typically visual disturbances like flashing lights, zigzag lines, or temporary vision loss. Some experience sensory changes, such as tingling in the hands or face, or even speech difficulties.
These episodes can be alarming, often prompting emergency room visits before the headache even sets in. Yet, despite the distinct clinical picture, the question remains: should treatment differ based on the presence of aura?
Current medical guidelines largely treat migraine with and without aura under the same umbrella. Acute therapies—like triptans, NSAIDs, and newer gepants—are commonly prescribed for both groups. Preventive options, including beta-blockers, topiramate, and CGRP monoclonal antibodies, are also used across the spectrum.
However, some experts argue that aura represents a different pathophysiological process. Aura is believed to result from cortical spreading depression, a wave of neuronal depolarization that may influence how certain medications work. For instance, there is a theoretical concern that triptans, which constrict blood vessels, could be less appropriate during the aura phase, though clinical data have not consistently supported this fear.
In practice, neurologists often tailor strategies based on individual symptom patterns rather than the mere presence of aura. A patient with prolonged or atypical aura may require additional diagnostic workup to rule out other conditions, such as transient ischemic attacks or seizure disorders. This can lead to different management choices, including the use of antiseizure medications as preventives.
Lifestyle factors also play a role. Some patients report that specific triggers—stress, certain foods, or hormonal changes—precipitate aura more than headaches without aura. Identifying and addressing these triggers can be as important as pharmacological intervention.

Preventive treatment, when needed, is generally selected based on the frequency and severity of attacks, not the aura status. However, emerging research suggests that certain preventive agents, like lamotrigine, may specifically reduce aura frequency, though evidence remains limited.
The medical community acknowledges a knowledge gap. Most clinical trials for migraine treatments do not stratify results by aura status, making it difficult to draw definitive conclusions. This has led to calls for more targeted research into aura-specific mechanisms and outcomes.
Patients with aura are often more disabled by their attacks, partly due to the unpredictability and the added sensory burden. This may warrant a more aggressive preventive approach earlier in the treatment course, but guidelines have yet to codify this.
For now, the consensus is pragmatic: treat the individual, not just the label. A thorough history, careful consideration of comorbidities, and regular follow-up are key to optimising outcomes for both migraine with and without aura.
Looking ahead, as precision medicine gains ground, we may see subtype-specific therapies emerge. Clinical trials that separately analyse aura and non-aura patients, along with advances in neuroimaging, promise to refine treatment strategies in the coming years.
